DVA COMMUNITY NURSING

The Community Nursing Program is administered by the Department of Veterans’ Affairs (DVA). It funds clinically required nursing and personal care, delivered in the home by registered and enrolled nurses and personal care workers, for eligible Gold and White Card holders who have a written referral from their GP or nurse practitioner.

Referrals are valid for 12 months and outline the services needed to meet the client’s assessed nursing needs. The client’s GP retains ongoing clinical oversight of their care throughout.

Clinical Nursing & Wound Care
Wound management and other clinical nursing tasks carried out by registered and enrolled nurses, in line with your care plan.

Medication Support
Help with managing and administering medication as directed by your GP.

Personal Care & Hygiene
Assistance with showering, dressing and daily hygiene for clients whose assessed needs require support.

Palliative Care Support
In-home nursing and personal care support as part of a palliative care plan, coordinated with your GP.

Eligibility: Gold Card and White Card

Gold Card holders can access community nursing services for any assessed clinical or personal care need, with a valid GP or nurse practitioner referral.White Card holders can access community nursing services where they relate to a condition accepted by DVA as war or service caused, with a valid GP or nurse practitioner referral.

Your GP or nurse practitioner writes a referral: the referral sets out the nursing or personal care services needed to meet your assessed needs.

We confirm eligibility and assess your needs: we check your card and referral details, then assess your clinical and personal care needs in person.

We develop a care plan with you: you receive a copy of your care plan, setting out the services to be provided.

Ongoing care, with your GP involved: we deliver the agreed services and your GP maintains clinical oversight, with referrals renewed every 12 months.
